Chronic Disease Management


  • Our goal is to improve patient self-care and awareness of optial diabetes care as well as preventing and delaying chronic diease-related complications of diabetes. All patients with Type II Diabetes receive an annual diabetes focus visit. Newly diagnosed or new patients with existing Type II diabetes will be referred for initial assessment.


  • The objective of Self Management of asthma is to obtain optimal asthma control and reduce frequency of asthma attacks. We will provide education and monitoring on asthma control, inhaler techniques, triggers to asthma, exercise induced asthma, and Action Plans through self management.


  • This program will outline the long-term effects of high BP and the positive effects lifestyle changes can have in controlling hypertension. We will provide BP screening for hypertension and teach you how to manage and control your own blood pressure.

High Blood Cholesterol

  • High cholesterol is a serious health problem that can block an damage your arteries, leading to cardiovascular disease and putting you at risk for a heart attack or stroke. The High Blood Chrolesterol Program will teach you how to make important diet and lifestyle choices that can reduce cholesterol in your system leaving you healthier and happier.

COPD (Chronic Obstructive Pulmonary Disease)

  • The Chronic Obstructive Pulmonary Disease (COPD) program aims to improve the health outcomes and quality of life for those who have been diagnosed with COPD through early detection and better management.
